Why Limit? - These forms often have very little to do with your specific treatment. The data is often used for research, profiling, government reporting and predictive analytics without your consent.
Choose - Use your best judgment as to what information, if any, you want to share.
Examples - The following are a few examples of questionnaires CCHF has received from patients, including one with patient responses we have chosen to show, as an example of what you can do.
